Care Navigation

Care navigation is the function, human, digital, or both, that routes a patient to the appropriate care setting, instead of the default most Americans fall back on: the emergency department. The category exists because the U.S. primary care system has a structural bottleneck: appointment wait times for new patients regularly run 31 to 90 days in many markets, and 167 million Americans delay care overall, 120 million specifically due to lack of adequate access. Care navigation vendors have proliferated to help employees make sense of that landscape, but most employer-sponsored navigation programs are advisory only, a nurse line or app tells an employee where to go, but the employee still has to get there, schedule it, and wait, and the underlying access problem is untouched.

This is the structural weakness in care navigation as a standalone benefit category: it can point at a destination without being the destination. The OnMed CareStation™ collapses navigation and resolution into a single encounter. Every walk-in is triaged by a Care Coordinator who collects chief complaint, history, and vitals before a licensed clinician deploys the CareStation's integrated diagnostic tools, and 86% of those visits are resolved on-site without a specialist referral, meaning the "navigation" step and the "care" step happen in the same twenty-minute encounter rather than as two separate events days apart.

For a self-funded employer, the ROI case is direct and immediate: 58% of CareStation users report they would have otherwise gone to an ED or urgent care setting had the CareStation not been available on-site, and the emergency department is roughly 12 times more expensive than an alternative care setting for the same non-emergent condition. Care navigation programs that only redirect volume, without changing what happens after the redirect, don't capture that cost differential, the savings only materialize when the destination itself resolves the visit. This is also why care navigation should be evaluated alongside population health management and chronic care management as a connected system rather than a standalone point solution: an employer buying navigation without resolution infrastructure downstream is paying for a better-organized version of the same access failure.
