A new OnMed CareStation® is now open inside a neighborhood grocery store, offering Fort Worth families no-cost primary and urgent care without an appointment.
OnMed has launched a new CareStation inside the Fiesta Mart at 421 West Bolt Ave in the La Gran Plaza shopping center, bringing tech-enabled, human-delivered healthcare directly into a store residents already visit every week. The CareStation opened to the public on Wednesday, July 9, 2025, at 10:00 a.m.
The Fort Worth CareStation is made possible through a grant from Molina Healthcare of Texas, whose support ensures the CareStation can deliver on-site, tech-enabled primary and urgent care at no cost to visitors. Molina Healthcare of Texas is part of Molina Healthcare, Inc., which serves Medicaid, Medicare, and Marketplace members across the state—making it a natural funding partner for a model built to reach people who are insured but underserved, as well as people with no coverage at all.
The July 9 opening was marked with a launch event at the store, bringing together representatives from Molina Healthcare of Texas, local community leaders, and OnMed's own team to introduce the CareStation to the neighborhood before it opened to shoppers. That kind of in-person introduction matters for a model like this one. A CareStation only works if the community trusts it enough to walk through the door, and a visible launch—with the people funding and running it standing next to it—helps build that trust.
Grocery stores are where communities already gather, which makes them a practical place to close a real gap in access. By placing the CareStation inside the Fiesta Mart on West Bolt Avenue, OnMed and Molina Healthcare of Texas are removing the two biggest barriers to routine care: cost and convenience. Shoppers can now connect with a licensed clinician the same way they'd pick up a prescription or check out a produce sale—no separate trip, no separate appointment, no further commute.
The OnMed CareStation® is an 8×10-foot "Clinic-in-a-Box" that delivers comprehensive, immediate care wherever people live, work, and shop. Inside, patients have a real-time video visit with a licensed physician, supported by diagnostic tools including thermal imaging, real-time scanning, and remote vital-signs monitoring. Each self-contained unit is designed to give patients the same personal, attentive experience they'd expect from a traditional clinic visit—private, secure, and staffed by a human clinician the entire time.

Fort Worth is one of five Texas communities with an operational CareStation, alongside Milam, Navarro, Bruni, and Missouri City—with a sixth, in McAllen, set to open later in July 2025. Nationally, OnMed has 30 locations of the CareStation under contract as it works toward a presence in 25 states by the end of the year. As trade coverage of the rollout has noted, Fiesta Mart is testing the "Clinic-in-a-Box" format across its stores as part of a broader push to bring everyday healthcare into the places people already shop.
Fort Worth's launch is a local story with a national pattern behind it. Roughly 80% of U.S. counties qualify as healthcare deserts, meaning residents face real barriers to basic medical care regardless of whether they have insurance. As grocery chains, health plans, and community organizations look for ways to close that gap without waiting years for a new clinic to be built, placing tech-enabled, human-delivered care inside a store people already trust offers a practical, fast way to get there.
